Cell phone/cancel service

I'canceling my service , called in to replace phone and I had moved. Customer service says I gave the wrong date of birth. i gave them the doc they did the data input and they cant get it right and the phone was so cheap. So I am canceling my service. Dont contact me because you have caused me a lot of grief and missed appointments. I am a disabled Vet and used that phone to contact my physicians.
