Changing of mode
Dear Sir, Policy No. 123127049, branch unit-12 R, old city 86, sector-16, faridabad
I have a policy in life insurance corpn. of india, due to some mistake my name write wrong in policy bond, I have try to correction in my wrong name before three months but there are no response, documents also submitted by me.
Now you are requested to please look into the matter and do the needful.

Changing of mode
Dear Sir,
I had applied for the L.I.C. Term insurance policy in the month of January 2012. All the formalities have been completed and all the relevant documents including the medical report and salary slips had been provided at the same time and everything was found satisfactory. The proposal no is 8153.Please find the decision report as an attachment for the reference. As per the decision slip, after everything was found satisfactory. Even along with the salary slip, I had deposited the bank statement as well.
The amount for the policy was debited from my account in the month of January 2012. However still I have not received the policy.
As per the L.I.C. Agent, the approval has not been received in their branch and he has already contacted the Branch office for the same as well. As per branch office, it has been dispatched from their side on 20th January 2012.

Being a consumer of the L.I.C., I have been suffering the pain. Even after paying the premium, still I am not able to get the benefit of the policy in terms of Tax saving and others.
I had paid Premium of 2 policies, one is for me and 2nd one is of my wife but has not received any of the policy.
Kindly look in to the matter and do the needful so that I could get the policy and its benefits ASAP.

non receipt of original policy bond
Dear sir, I Shri Ashim Kumar Mukherjee submit my policy bearing no 411055177 to your Naihati Branch (Branch code 435) for Reassignment on dated 18.07.2011 but unfortunately till today the original policy not handed over to me. Regarding the same matter I personaly mate your branch manager on 20.10.2011 and he asked me to lodge a complain about the same accordingly I do the same on dated 11.11.2011 but no communication has been made from the said branch.Now please help me to gate me back the reassinged original policy bond.
